Huron community Family Health Team is dedicated to providing a full range of health services with a focus on prevention of illness and chronic disease management.
Family Physicians will provide continuous care to individual patients and their family members over the course of their lifetime.
Family Physician
Nurse Practioner
Nurse Practitioners will provide the opportunity to improve the individual’s, family’s and community’s direct access to a broad range of primary healthcare services, with a strong focus on nursing.
Physician Assistant
Physician Assistants are highly skilled healthcare professionals educated in the medical model who work under the supervision of a registered physician in a variety of clinical team structures and settings.
Social & Mental Health
Mental Health Clinicians
Mental Health clinicians will provide direct care through the provision of psycho-social assessments and individual, couple, family, and group counselling. They will collect patient information and communicate findings with other healthcare professionals of the FHT.

Registered Dietician
Registered Dieticians are healthcare professionals who apply their unique body of knowledge and expertise to support people in understanding and applying the principles of healthy eating throughout their entire lifetime. As members of an interdisciplinary team, dieticians can contribute to health promotion and illness prevention strategies, and develop specialized nutrition therapy and rehabilitation/support strategies to address specific nutrition-related illnesses.

Memory Clinic
The aim of this clinic is to help patients with memory difficulties in order to maintain independent living and the best quality of life for as long as possible. Testing done by staff at HcFHT is done in partnership with the Alzheimer Association. Mind, Body, Medicine
This group practices mindfulness using proven techniques to bring about inner peace. These techniques are also effective in reducing symptoms of mood and anxiety problems. New Parent Programs
New Parent Programs continue to run during COVID-19.
Introduction to Solids
This program will provide recommendations from the World Health Organization about breastfeeding and introduction of complementary foods by our Registered Dietitian and RPNs. The program also discusses what and how to feed your baby when introducing solid foods and food safety. Those interested in the program do not have to be a patient at HcFHT and do not require a physician or healthcare provider referral.

My Journey
The My Journey program assists youth struggling with mental health concerns and/or addiction. Help is provided for both youth and their families through system navigation support throughout the duration of their treatment.
